Notes:  

(1) I do not have a 3-string capo, so this transcription was only possible
because I do have a VG-8 and I could tune up the three top strings without
breaking them.    

(2) I have no idea what it is like to play with a 3-string capo!  It would 
have the effect of keeping the three capoed strings where they are for any 
fretting done on those strings at the 5th (impossible because of the capo) 
or below frets, and the effect of being at fret zero for the capoed
strings starting from fret 5 and above.  Does the capo get in the way -
make it hard to do the chord shapes?  I have no idea!

(3) Jonatha says the tuning is:

Tuning:  CGDGBbD + 3-string capo on the 5th fret
         
(4) I used the tuning of 

CGDGBD (I changed the Bb of the second string in Jonatha's tuning to B)

as the basis, then used the VG-8 to tune up each of the top three strings 
by 5 half-tones.  The resulting actual tones of the strings after tuning 
up are then:

VG-8 pitches:  3 5 5 0 -2 -4  (left to right, highest to lowest strings)
